A toxin test can be paid or reimbursed from an HSA or FSA only if it counts as a qualified medical expense. IRS rules cover the costs of diagnosis, cure, mitigation, treatment or prevention of disease, and exclude expenses that are merely beneficial to general health.[[1]](#source-1) Whether a particular test qualifies depends on why it is done and on your plan’s rules, and some administrators ask for a letter of medical necessity from your clinician.[[2]](#source-2)[[3]](#source-3) A seller’s “HSA/FSA eligible” label is not a guarantee that your claim will be approved.[[4]](#source-4)

## The short answer

**Yes, it’s HSA/FSA eligible.** The Intrinsic Prism test is HSA/FSA eligible. You pay at checkout, then claim reimbursement with the itemized invoice we provide. Your plan administrator decides whether the claim is approved.

Whether HSA or FSA funds can pay for a toxin test depends on IRS rules for medical care and on your plan. Intrinsic Prism (**$1,999 per participant**) is HSA/FSA reimbursable with an itemized invoice; your plan administrator decides. Keep the invoice and check your plan's rules, including whether it needs a letter of medical necessity.

## What the IRS rules say

- **Medical care, not general health.** IRS Publication 502 defines medical expenses as costs of diagnosis, cure, mitigation, treatment or prevention of disease, and says they don’t include expenses that are merely beneficial to general health.[[1]](#source-1)
- **Laboratory fees.** Publication 502 lets you include laboratory fees that are part of medical care.[[1]](#source-1)
- **HSAs and FSAs follow the same idea.** Publication 969 says qualified medical expenses for these accounts are those that would generally qualify for the medical expense deduction, and for FSAs, those specified in the plan.[[2]](#source-2)
- **Diagnosis is the key word.** The IRS says a physical exam qualifies because it provides a diagnosis of whether a disease or illness is present, while nutritional counseling qualifies only if it treats a specific disease diagnosed by a physician.[[5]](#source-5)

## Letters of medical necessity

Plan administrators use a letter of medical necessity (LMN) for items that fall between medical care and general wellness. HealthEquity’s form says some services are only eligible when your doctor certifies they are medically necessary, naming the diagnosed condition, the treatment and its length.[[3]](#source-3) HealthPartners notes that a letter does not guarantee reimbursement and must be renewed each plan year.[[4]](#source-4) Whether a toxin test needs an LMN is up to your administrator.

## What test sellers say (and what that means)

Published HSA/FSA statements, checked October 22, 2026. These are the sellers’ own words, not a determination by your plan.

## How to check before you buy

1. Ask your HSA or FSA administrator whether a laboratory toxin test is a qualified expense under your plan, and whether it needs a letter of medical necessity.[[3]](#source-3)
2. If you have a diagnosed condition your clinician wants to investigate, ask whether they will document the medical reason.[[5]](#source-5)
3. Keep an itemized receipt. For HSAs, the IRS requires records that show distributions paid qualified medical expenses.[[2]](#source-2)

Limitations: a single sample is a snapshot of recent exposure; urine results depend on hydration and timing, which is why labs adjust for creatinine; reference ranges for many chemicals come from population surveys such as CDC's NHANES, not disease cut-offs; and urine mycotoxin testing is still debated.
